Step 1 of 3
Request your records
Fill in the boxes and your request is written beside them. It asks for a copy of your records under Article 15 of the UK GDPR (a “subject access request”). It is free in most cases and the organisation should reply within one month. You copy, print or download it and send it yourself.
Information and templates, not legal advice
Check the records contact on the organisation’s own website (often the practice manager, medical records team or data protection officer). What you type is saved only in this browser.
Your request
Read it through before you send it and keep a copy. They may ask for photo ID and proof of address. Note the date they receive it: the usual deadline is one month.
When your records arrive, go to step 2.
